Through MyHealth:<\/strong><\/p>\n<ul>\n<li>Log in and visit the <a href=\"https:\/\/myhealth.lhs.org\/MyHealth\/app\/sharing-hub\/\" target=\"_blank\" rel=\"noopener\">Sharing Hub<\/a>.<\/li>\n<li>Choose what you need:\n<ul>\n<li>Download a copy for yourself.<\/li>\n<li>Request a formal copy for official purposes.<\/li>\n<li>Share records electronically with family\/caregivers.<\/li>\n<li>Grant access to health apps.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>2. Online Form:<\/strong><\/p>\n<ul>\n<li>Fill out the <a href=\"https:\/\/intake.doctible.com\/new\/v9jjpx\" target=\"_blank\" rel=\"noopener\">Release of Health Information<\/a> form.<\/li>\n<li>This allows Metropolitan Pediatrics to:\n<ul>\n<li>Get records from another healthcare provider.<\/li>\n<li>Release records to another provider or person.<\/li>\n<li>Share patient information with a school or daycare.<\/li>\n<\/ul>\n<\/li>\n<li>Important: Use the form only if you need records or school communication within the next 30 days.<\/li>\n<\/ul>\n<p>For help or to check on a previous request, contact the Health Information team Monday through Friday, 8 am to 5 pm. You may reach them by phone at 503-601-3417, fax at 503-466-1858, or email at <a href=\"mailto:HIS@metropediatrics.com\">HIS@metropediatrics.com<\/a>.[\/vc_column_text][\/vc_column][\/vc_row][vc_row el_class=&#8221;pt60 mpt20&#8243;][vc_column width=&#8221;5\/12&#8243;][vc_column_text]<\/p>\n<h3>MyHealth FAQs<\/h3>\n<p>Learn how to use MyHealth to schedule appointments, print immunization records, make payments for medical bills, and more.[\/vc_column_text][\/vc_column][vc_column width=&#8221;7\/12&#8243;][vc_toggle title=&#8221;How do patients sign up for a MyHealth account?&#8221;]To sign up for MyHealth, the patient must be at least 14 years old. They can either <a href=\"https:\/\/myhealth.lhs.org\/MyHealth\/accesscheck.asp\" target=\"_blank\" rel=\"noopener\">sign up online<\/a> or in person with our team.[\/vc_toggle][vc_toggle title=&#8221;How do parents access their child\u2019s MyHealth account?&#8221;]Parents, legal guardians, healthcare representatives, and foster parents can <a href=\"https:\/\/intake.doctible.com\/new\/u0btuk\" target=\"_blank\" rel=\"noopener\">submit a request for proxy access<\/a> to a patient&#8217;s MyHealth account. <strong>If a patient is 14 years old or older, they have the right to privacy for their own account and must approve the request for another party to review their records.<\/strong><\/p>\n<p>A proxy can see the patient&#8217;s medical records, immunizations, and test results. They can also message their care team and schedule appointments for them.<\/p>\n<p>Once the proxy request is submitted, our team will review it. If the request is approved, the proxy will be granted access to the patient&#8217;s account. The proxy can then log into the patient&#8217;s account using their own username and password.<\/p>\n<p><strong>It&#8217;s important to note that proxy access to a patient&#8217;s MyHealth account is automatically disabled on two occasions:<\/strong><\/p>\n<ul>\n<li><strong>The patient&#8217;s 14th birthday. <\/strong>Oregon law gives teens the right to privacy over their health information once they turn 14. Proxy access can be re-enabled with the patient\u2019s consent.<\/li>\n<li><strong>The patient&#8217;s 18th birthday.<\/strong> Adults have the right to privacy over their health information. Proxy access can be re-enabled with their consent.<\/li>\n<\/ul>\n<p>To re-enable proxy access, complete the <a href=\"https:\/\/intake.doctible.com\/new\/u0btuk\" target=\"_blank\" rel=\"noopener\">request form<\/a> and we&#8217;ll contact the patient to get their approval. Patients can also share their MyHealth account with another person following the instructions below.[\/vc_toggle][vc_toggle title=&#8221;How do patients invite someone to access their MyHealth account?&#8221;]<\/p>\n<ol>\n<li>Go to the <a href=\"https:\/\/myhealth.lhs.org\/MyHealth\/Proxies\/FamilyAccess\" target=\"_blank\" rel=\"noopener\">Friends and Family Access<\/a> page.<\/li>\n<li>Click <strong>Invite Someone<\/strong> under the \u201cWho Can See My Record?\u201d section.<\/li>\n<li>Enter that person&#8217;s name and email address, confirm the level of access, and click<strong> Send Invite<\/strong>. The invitation then appears as \u201cPending\u201d at the top of the page.<\/li>\n<li>Once an invitation is sent, the recipient will receive an email notification. This email will include a link for them to click to verify that they know the patient and accept the invitation.<\/li>\n<li>Once the invitation is accepted, the patient will receive a notification. The &#8220;Pending&#8221; label will disappear next to the person\u2019s name on the Friends and Family Access page.<strong> Patients can edit or remove someone\u2019s access at any time by returning to this page.<\/strong><\/li>\n<\/ol>\n<p>[\/vc_toggle][vc_toggle title=&#8221;Can I access my MyHealth account from outside the United States?&#8221;]Unfortunately, no. Due to different privacy regulations in other countries, MyHealth isn&#8217;t accessible outside the US. However, there are still ways to get the information you need:<\/p>\n<ul>\n<li><strong>Medical Records:<\/strong>\u00a0Submit an <a href=\"https:\/\/intake.doctible.com\/new\/v9jjpx\" target=\"_blank\" rel=\"noopener\">online request<\/a> for records.<\/li>\n<li><strong>Immunization Records:<\/strong>\u00a0<a href=\"mailto:HIS@metropediatrics.com\">Contact us<\/a> and we&#8217;ll send you a copy.<\/li>\n<\/ul>\n<p>Important: Patients 14 and older must approve requests to release their medical records, except for immunizations (accessible to parents\/guardians of patients under 18).[\/vc_toggle][vc_toggle title=&#8221;How do I schedule an appointment?&#8221;]To schedule an appointment, follow these steps:<\/p>\n<ol>\n<li>Switch over to the patient&#8217;s MyHealth record.<\/li>\n<li>Go to <strong>Find Care &gt; Schedule an Appointment<\/strong>.<\/li>\n<li>Choose the provider or appointment type you want and select an appointment time that works for you.<\/li>\n<\/ol>\n<p>Appointments can only be scheduled with a Metropolitan Pediatrics provider that the patient has seen previously. To schedule an appointment with a new provider, you can either <a href=\"https:\/\/www.metropediatrics.com\/appointments\/\">book a visit<\/a> on our website or call us at 833-PDX-KIDS.[\/vc_toggle][vc_toggle title=&#8221;How do I cancel or reschedule an appointment?&#8221;]Here are the steps to cancel or reschedule an appointment through MyHealth:<\/p>\n<ol>\n<li>Switch over to the patient&#8217;s MyHealth record.<\/li>\n<li>Go to <strong>My Record &gt; Visits<\/strong>.<\/li>\n<li>Select the appointment you want to cancel or reschedule from the &#8220;Upcoming Visits&#8221; list.<\/li>\n<li>If you want to cancel the appointment, click <strong>Cancel appointment<\/strong>, enter any comments, and click <strong>Confirm cancellation<\/strong>.<\/li>\n<li>If you want to reschedule the appointment, click <strong>Reschedule appointment<\/strong> and select a new appointment time that works for you.<\/li>\n<\/ol>\n<p>If the appointment is coming up soon, you might not be able to cancel it through MyHealth. In that case, you&#8217;ll need to call the clinic to cancel.[\/vc_toggle][vc_toggle title=&#8221;How do I eCheck-in for an appointment?&#8221;]Three days before an appointment, the patient or their proxy will receive an email notification with a link to start the check-in process. Click the link to pay the visit copay, update the patient\u2019s information, and complete any required health screeners. This will help us get started right away when your family arrives for the appointment.<\/p>\n<p>If you can&#8217;t find the email, you can log into MyHealth and go to the <a href=\"https:\/\/myhealth.lhs.org\/MyHealth\/Authentication\/Login?postloginurl=Visits\" target=\"_blank\" rel=\"noopener\">Appointments and Visits<\/a> page, then select eCheck-in for your appointment.[\/vc_toggle][vc_toggle title=&#8221;How do I view and print an immunization record?&#8221;]<\/p>\n<ol>\n<li>Switch over to the patient&#8217;s MyHealth record.<\/li>\n<li>Go to <strong>My Record &gt; Health Summary &gt; Immunizations<\/strong>.<\/li>\n<li>You\u2019ll see a list of all the immunizations the patient has received.<\/li>\n<li>To print a copy of the patient&#8217;s immunizations, click on the <strong>printer<\/strong> icon.<\/li>\n<\/ol>\n<p>[\/vc_toggle][vc_toggle title=&#8221;How do I request a medication refill?&#8221;]<\/p>\n<ol>\n<li>Switch over to the patient&#8217;s MyHealth record.<\/li>\n<li>Go to <strong>My Record &gt; Medications<\/strong>.<\/li>\n<li>Click <strong>Request Refill<\/strong> below the medication.<\/li>\n<li>Select a delivery method, pharmacy, and pickup date and time that&#8217;s convenient for you, if applicable. Click <strong>Next<\/strong>.<\/li>\n<li>Review the details of the refill request and click <strong>Submit<\/strong>.<\/li>\n<\/ol>\n<p>You\u2019ll receive a MyHealth message when the refill request has been sent to the pharmacy.[\/vc_toggle][vc_toggle title=&#8221;How do I update insurance information?&#8221;]To review the insurance information Metropolitan Pediatrics has on file, go to <strong>Insurance &gt; Insurance Summary<\/strong>. On this page, you can update the patient\u2019s coverage, replace insurance card photos, remove coverage, or add new coverage.[\/vc_toggle][vc_toggle title=&#8221;How do I pay a bill or set up a payment plan?&#8221;]<\/p>\n<ul>\n<li><strong>If you have a MyHealth account<\/strong>, you can <a href=\"https:\/\/myhealth.lhs.org\/MyHealth\/Billing\/Summary\" target=\"_blank\" rel=\"noopener\">log in and pay your bill<\/a>.<\/li>\n<li><strong>If you don&#8217;t have a MyHealth account<\/strong>, you can <a href=\"https:\/\/myhealth.lhs.org\/MyHealth\/billing\/guestpay\/payasguest?&amp;mobile=0\" target=\"_blank\" rel=\"noopener\">pay as a guest<\/a> with the account number on your statement.<\/li>\n<\/ul>\n<p>If you&#8217;re unable to pay the entire balance in full, you can set up a payment plan to pay a smaller amount each month.[\/vc_toggle][vc_toggle title=&#8221;Is there a MyHealth app?&#8221;]Yes, MyHealth has a mobile app called MyChart.
