New patients can call our office, book an appointment online, or fill out an online form to receive a callback. Our team will then guide you through completing registration forms, transferring medical records, and scheduling your child’s first visit.
How do I find the clinic?
We are located at 15455 NW Greenbrier Parkway in Beaverton, near the Tualatin Hills Athletic Center and just off US-26 at Exit 65 (NW Cornell Road). You’ll find us in Suite 111 on the ground floor, with free parking directly outside and step-free ADA accessibility.
