We’re delighted to welcome Dr. Eman Lutfi to our Beaverton team this April. If you’re one of her current patients and would like to continue seeing her at Metropolitan Pediatrics in Beaverton, we’re happy to help you move to our office in three easy steps!
- First, please complete this registration form. This lets our team know that you’re joining us, so you’ll have access to Dr. Lutfi, your care team, 24/7 nurse advice, prescription refills, appointment scheduling, and all the other things you need to keep your kiddo(s) healthy. We’ll give you a call to finish setting your family up. Please review our Treatment Consent, Billing & Financial Policies, and Notice of Privacy Practices before signing the form.
- Second, if you have not already done so, please give us permission to request your child’s medical records from their previous office.
- Third, we’d love to set you up with a patient portal account, so you can access your child’s care team and health information securely, anytime. To access your child’s record on MyHealth, you’ll need to request proxy access. If your child is 14 or older, they will have to sign this form and agree to give you proxy access.
