If you would like to be considered for reduced monthly payments or discounts, please complete a Financial Assistance Application and submit it to our Revenue Cycle team for review. If you are experiencing job loss, divorce, or other extenuating circumstances, please indicate this on the application.
Include the following items for consideration:
- Copy of last year’s tax return
- If you did not file taxes, you must submit a letter from the IRS stating that you did not file a return.
- To obtain a letter, contact the IRS at 1-800-829-1040.
- Three (3) most recent pay stubs, including spouse (if applicable)
