What is MyHealth?
MyHealth offers patients convenient and secure online access to their personal health information.
With MyHealth, you can:
- Request appointments
- View test results
- Request prescription refills
- Access trusted health information resources
- View your health summary from the MyHealth electronic health record
- Communicate electronically with your Metropolitan Pediatrics Care Team

How do I sign up?
Patients who wish to participate will be issued a MyHealth access code during their clinic visit. This code will enable you to log in and create your own user ID and password.
If you were not issued an access code during your visit, you may request one online.
Can I access a family member’s MyHealth account?
Yes you can. This is called proxy access and allows a parent (or guardian) to log into their personal MyHealth account, and then connect to their family member’s account. Patients 14 and older must give written permission for a parent or legal guardian to have access and/or they can have their own account.
To request proxy access, please complete and return the MyHealth Proxy Access Request form to your clinic, or fax to Health Information Services at 503-466-1858.
