RN – Care Management – Gresham
The RN-Care Management will provide Care Management for our Gresham location. Responsibilities include:
- Developing and maintaining a registry of patients with special healthcare needs, and in conjunction with physicians, developing, coordinating and implementing care goals for these patients.
- Educating and presenting to patients, families and peers.
- Providing triage for physicians at site and patient care services if necessary.
- Scheduling appointments as necessary, and updating the Electronic Medical Record (EMR) with relevant information.
The following is required:
- Understanding of clinic workflow.
- Good interpersonal, communication, telephone, customer service, time management, and decision-making skills. Ability to work in a team environment and ability to multi-task essential.
- Pediatric care management experience.
- Current RN license in the State of Oregon.
- Degree from an accredited Nursing program with a focus on Pediatrics. Current CPR certification.
- 1-2 years experience in a family practice or pediatric clinical setting, coordinating care, giving advice and performing triage.
- PALS certification preferred.
This position is 40 hours a week, Mondays - Friday. We offer a competitive wage in a team-oriented, flexible work environment.
